About Your First ENT Visit

With nearly 25 years serving the families of Valdese, NC, our Board Certified ENT physician, Dr. Jones, enjoys seeing each current patient and especially our new patients to begin building a trusted and long term relationship from the very first visit. Our dedicated staff requests you to please bring the following items to your first visit:

  • Health insurance card/information
  • A list of all medications you are taking
  • A list of your questions/concerns
  • A copy of your XRAYS/CT scans (reports and actual films)
  • Copies of records from doctors related to your sinuses/nose/allergies/lungs
  • Completed forms provided below

Scheduling Appointments

When you schedule an appointment at the Ear, Sinus & Allergy Center, we set aside that designated time just for you. Out of respect for your time and to further develop our patient-physician relationship, we do not double-book appointments or rush through exams. We want to take the time to get to know you, your symptoms and what treatment options are best for you in order to provide you with the best comprehensive family ENT care in Valdese, NC. We want to be sure that when you leave your consultation with Dr. Jones, you are comfortable with the treatment plan he has outlined as well as have all of your questions answered.

We strive to stay on time every day. To help us accomplish this, please arrive 20 minutes early to complete paperwork on your first visit. To speed up the check-in process, we request you to visit our Patient Portal to get your paperwork started before even arriving. After your first visit, little or no additional paperwork will be needed.

If you cannot attend your appointment, please let us know as soon as possible. As we do not double book patients, this time is set-aside solely for you. Please allow us to see another patient in need if you are unable to attend.

If you are late for your appointment, please be aware that we will do our best to see you as quickly as possible; however, we cannot make other patients wait for their appointments.

Financing Options Available Through CareCredit

CareCredit offers a full range of payment plans. The 3, 6, 12, & 18 month No Interest* payment plans feature monthly payments as low as 3% of the balance and let you pay within the specified time without incurring interest charges. Simply make your minimum monthly payment and pay off your entire balance during the agreed upon time period, and you’ll pay no interest.

For patients/clients with higher treatment fees who would prefer more time to pay with lower monthly payments, our 24, 36, 48, & 60 month extended payment plans* offer a low, fixed 13.9% interest rate and an extended term.

You could get a $2,500.00 procedure for as low as $139.00 a month, for 18 months, with no interest. Or, you could have the same procedure and only pay $60.00 a month, for 60 months, plus interest.

*No interest if paid within 3, 6, 12, 18 or 24 Months* (Minimum monthly payments required)

*Valid on purchases of $300 or more made on your CareCredit account. On promo purchase balance, monthly payments required, but no Finance Charges will be assessed if (1) promo purchase balance paid in full in 3,6,12,18 or 24 months, (2) all minimum monthly payments on account paid when due, and (3) account balance does not exceed credit limit.
